Introducing the Comp Corner, providing you with
continuous general information on your Workers
Compensation benefits.
The Open Enrollment Period ended on December 14,
2007 and all requests for changes have been
processed. The next Open Enrollment will be in
November 2008 with implementation of changes
effective January 2009. Please remember The
only other time during the year that you may make
changes to your coverage would be due to certain,
limited life events (i.e. marriage, divorce,
birth, other insurance coverage, etc.) If want
to make a coverage change and are unsure if you
have had a lifes event, please contact our

In recent months the Benefits Office has
experienced employees who notify us of a
work-related injury after they have already
sought treatment from an unauthorized
physician. Please be aware that you should
notify our Office and complete a First Report of
Injury or Illness prior to visiting a physician
for an illness or injury you may have sustained
at work. Failure to do this may result in your
visit not be covered by Workers Compensation and
the claim will not be covered under your health
insurance coverage if it was a work-related
injury or illness. Please visit our website at

ATTENTION!
Effective January 1, 2007, the Individual
Calendar Year Deductible of 300.00 will be
waived in situations where the Participants
Spouse or Domestic Partner, who is also an
employee of an Employer, has elected family
coverage under the Plan. If you qualify for this
benefit and would like your deductible waived or
if require further explanation of this benefit,
please contact our office at [phone].
Provider Networks for Medical Coverage
In our last issue, we identified the three
networks available under your medical coverage
Keys Physician Hospital Alliance (KPHA),
Dimension Health, and Multiplan. If you find
that a physician you would like to visit is not
in the network, the following can be done
For Keys Physician Hospital Alliance (KPHA)
please call 1-800-400-0984 or 305-294-4599 to
nominate a physician.
For Dimension Health a provider can visit
www.dimensionhealth.com and complete an
application by clicking on Provider Application
located on the left hand side of the website.
The application must be completed and mailed into
the address provided for Dimension Health.
For Multiplan a provider can contact Multiplan
by email at newprov_at_multiplan.com to express
interest in joining the network.
