short-term assistance in the return of a offender
to Southeast Fort Wayne. - Provide a list of available neighborhood
resources to facilitate the reintegration of the
offender to the neighborhood.

- L/E Team members meet with the offender
pre-returnID transitional housing needs - RVT contacts identified landlords or housing
contacts to aid in securing housing - Within 48 hours of return L/E members of the team
meet with the offender to review rules,
expectations, and initiate dialogue regarding
employment, education, reuniting with family, and
spiritual needs - NLOs or district police officers will meet with
the offender every several days to monitor
progress continuing mentoring role

- First two weeks of the offenders return, the
neighborhood association president and nearby
residents have been informally monitoring the
offenders behavior - Week three, entire RVT meets with the offender.
- Neighborhood association president welcomes the
offender to the neighborhood and encourages the
offenders involvement in neighborhood activities - Neighborhood specialist provides list of
available community services

Duration and Coordination of RVT Activities
- The duration of Re-entry Visitation Team
activities will be coordinated with the case
manager of Allen County Community
Corrections(ACCC) for the particular returning
offender. All RVT activities will cease once the
offender has successfully completed the Re-entry
Courts program.

Duration and Coordination of RVT Activities
During the monitoring period for RVT Team
members, adverse contacts in the community,
observations of behavior by the returning
offender that are in violation of ACCC rules or
violations of the criminal law will be reported
to the offenders case manager as soon as
possible. Routine meetings with the case manager
will occur once per month.

Outcomes or Results
- The desired outcome of this project is to
- enhance the services provided to the re-entry
offender - encourage participation in community activities
- aid in the network that will support successful
re-entry to Southeast Fort Wayneand, - encourage informal monitoring of of the
offenders progress. - Two measures, participation in community
activities and recidivism, will define the
success of the Re-Entry Visitation Team

Conclusion
- The Re-Entry Visitation Team is designed to
provide a supportive environment as a returning
offender attempts to engage in a positive and
productive life in our community. We believe that
community involvement, firm and realistic
expectations, assistance, and monitoring of
behavior will make a difference in the lives of
the willing offender.
