Security Affidavit

• Please
• Do NOT disclose contents of the tests to any
  person in any fashion
• Do NOT copy any part of the tests / materials
• Keep tests secured until distributed to students
• Limit access to tests/materials to actual testing
  periods (do not move on or skip sequence / time
  period
• Collect, account for materials after each test
  period (tests/materials are NOT to be removed
  from room by students)
• Do NOT review any part of the tests with anyone
  before, during, and after testing
• Do NOT develop scoring keys or review or score
  student responses
• Return all test materials to STAR test site
  coordinator daily (AP Office)
• Administer tests as instructed in the direction /
  verbatim
• Sign and return Affidavit to test site
  coordinator after training session

• Read Directions in the SAY Box!
• Verify each student has his/her correct Response
  Booklet!
• Verify that you have collected all test booklets
  at the end of the testing session
• Be sure that Box 1 is completely filled out
• Bubble Sections A3 and A4 after student has
  tested and has used the accommodation or
  modification
