What is screening?
  • Looking for disease in the absence of symptoms
  • Remember First do no harm
  • Wilsons criteria have been used to test the
    validity of screening tests

False positives (1)
  • ie saying theres something wrong when there
    isnt
  • A common problem with screening tests eg PSA
  • Creates enormous anxiety and a need for further
    tests

  • Either saying theres a problem when in reality
    there isnt ie technical issue/reading error etc
    (about 3 in every 20 normals are erroneously
    called abnormal)or
  • Finding a real abnormality which is irrelevant ie
    incidentalomas or pseudodisease

  • Has been estimated that about 1 in 20 cases of

  • Taking all the possible outcomes into account,
    made the following estimate
  • There are between 5-10 times as many individuals
    suffering detriment.

Other issues (1)
  • It may be the wrong test
  • The NHS picks up the tab
  • Concerns about radiation dose
  • COMARE quote If scanning is repeated every year
    to 5 years from 40 onwards, it would lead to a
    high cumulative radiation exposure which could
    lead to 1 in 50 of those individuals so exposed
    dying from a malignancy induced by the CT
    scanning.

  • Tests without a context dont make sense eg
    stress testing
  • It over-values screening and underemphasises the
    importance of symptoms
  • Sold as empowering
  • There is no good scientific evidence that CT
    screening of individuals is a worthwhile exercise
