What happened 2 personnel were attempting to
tighten the Top Drives top side swivel nut using
a drive pin tool and 8 lb sledge hammer while
working from the Man Lift. When EE 2 swung the
hammer, the handle contacted the top rail of the
Man Lift basket, directing the hammer head upward
into the finger of the EE 1 who was holding the
pin driving tool. The incident resulted in a
fracture and burst wound to the tip of the
left-middle finger.

• Learning points
• Wrong tool for the job Manufacturer has a tool
  available, specifically designed to break out /
  tighten the swivel packing nuts.
• Mentality This is how weve always done the
  job.
• HAZID Difficulty swinging a hammer in cramped /
  limited work space.
• HAZID Potential contact with other objects
  while swinging hammer.
• JSA did not identify hazards associated with
  working with heavy tools swinging hammers from
  the Man Lift Basket.
• Current Corrective Actions are for field
  designed tool not to be used and Contractor
  Supt. or Top Drive rep. to supervise / assist
  with field servicing of swivel packing.
