DESCRIPTION OF INCIDENT

• On 2008/09/17 at about 1435.
• Ten 26 gangsters attacked other offenders during
  the exercise time.
• A fight erupted.
• Unit members they got held up in the middle of
  the fight and could not handle the situation.
• Help was summoned.
• Members disarmed the offenders.
• Offenders that were fighting back calls
  themselves the Forums / Peace Committees.

IMMEDIATE PRECAUTIONARY MEASURES

• All offenders in the Centre were locked up.
• Movement of offenders were restricted.
• Meeting with all staff conducted on 18 September
  2008.
• Addressed by the Area Commissioner.
• Head of Correctional Centre implemented various
  security measures from the Contingency Plan.
• Searching of the Correctional Centre took place.
• Mass searching took place on 25 June 2008 and
  26 June 2008 by the officials of the EST.
• A mass searching of the Centre is planned.

ANTICIPATED DISCIPLINARY MEASURES

• The matter was handed over to the SAPS at
  Westville.
• Four officials were appointed as Investigators,
  namely
• Mr. J Stuart (DD)
• Mr. B Lambert (ASD)
• Dr. G Govender (SCO)
• Mr. S Mthiyane (COG I)
