Fire Safe Cigarette Registration Process

• Provided are instructions to use our online
  application to register your compliance with PA
  56 of 2009, known as the Fire Safety Standard and
  Firefighter Protection Act.
• Cigarette manufacturers are required to register
  their cigarette brand families and pay a fee of
  1250.00 for each brand family in order to sell
  these cigarettes in Michigan after January 1,
  2010.
• Cigarette manufacturers are also required to
  submit proof of their approved FSC packaging mark
  and laboratory certification for each cigarette
  style. These two documents should be submitted
  by email to statefiremarshal_at_mighigan.gov prior
  to using this online application. If you have
  not already submitted your FSC packaging mark and
  laboratory certification please do so before
  entering your brand family and style information.
  You will receive a confirmation email from the
  State Fire Marshals office advising you that the
  mark and lab reports have been accepted and that
  you may proceed with registration.
• As your registration information is reviewed and
  approved you will receive a confirmation email
  that provides you with a Permanent ID number for
  the Brand Family and any Cigarette Styles
  registered with us. You should use Permanent ID
  number when requesting any changes to your
  records, or when contacting the Bureau to discuss
  your records.

• Create a User Account for each Brand Family of
  Cigarette
• Go to Register on the Menu
• Click the Register Link

New User Register and Create an Account

• Brand Name Enter the name of the Cigarette Brand
  Family
• Manufacturer Name Enter the Manufacturer Name
• Address Enter the street address of the
  Manufacturer
• Daytime Phone Enter a daytime phone number for
  the contact person at the Manufacturer
• Fax Enter an optional fax number for the
  Manufacturer
• Email Enter the email address for the contact
  person at the Manufacturer
• User ID Create a User ID to allow access to add
  cigarette styles for this Brand Family
• Password Designate a Password for this account
• Click the Register Button (You will then be
  promoted to login to the application using the
  UserID and Password you just created)
• If you forget your UserID or Password send an
  email to statefiremarshal_at_mighigan.gov. Provide
  the Brand Family Name and the Permanent ID number
  if possible.

• You must register the Brand Family before
  registering any cigarette styles
• Click the Register Brand Family menu item

• From the Profession list select Fire Safe
  Cigarettes
• From the License Type list select Cigarette Brand
  Family
• From the Obtained by Method list select
  Application

• If you have not already submitted your FSC
  Markings and the copies of the lab tests for each
  of the cigarette styles you intend to register
  please contact the Bureau of Fire Services
• Click Continue to Next Step

• Enter the name of the authorized person who is
  submitting the information
• Click the eSignature button

• Review the information you submitted at the
  bottom of the page
• If you need to change the answer to any questions
  use the Questions link on the left.
• If you are ready to submit the record click the
  Pay Fees and Submit button

• You will be assessed a fee of 1,250.00 per Brand
  Family.
• Click the Submit Payment button to continue and
  process your payment.

• The following information is required on this
  page
• Billing Name
• email address
• the name on the credit card
• credit card number
• expiration date
• CVV code from the back of the card.
• Click the Next button after you have entered
  the required information.

• Verify your billing name and address and that you
  have been properly assessed the 12,50.00 fee.
• If the information is correct click the Pay Now
  button.

• Print this page if you wish to retain a copy of
  your payment transaction.
• Your registration is not complete until you also
  individually register each cigarette style that
  will be sold in Michigan after January 1, 2010.
  You can do that in separate sessions, however
  your records are not complete until the styles
  are registered.
• To begin adding cigarette styles click the Add
  Style button.

• From the Profession list select Fire Safe
  Cigarettes
• From the License Type list select the style. If
  you have a style that is not listed please
  contact statefiremarshal_at_michigan.gov to have the
  style added
• From the Obtained by Method list select
  Application
• Click the Continue button

• When you are ready to enter style information
  click the Continue to Next Step button

Add a Cigarette Style

• FSC Style Name is used to uniquely identify a
  cigarette style within a Brand Family. Use a
  combination of the Brand Family Name and the
  style name (i.e., Ace Full Flavor 100s)
• FSC Flavor is used to identify the flavor of the
  cigarette, if applicable (i.e., Menthol,
  Chocolate, etc)
• FSC Filter Type is used to identify type of
  filter on the cigarette and is a required field
• FSC Package Type is used to identify the type of
  package for the cigarette style and is a required
  field.
• FSC FDA Descriptor is not required currently
• FSC Length is used to identify the length of the
  cigarette and is a required field
• FSC Circumference is used to identify the
  circumference of the cigarette and is a required
  field
• FSC Third Party Lab Name is used to identify the
  name of the lab performing the RIP testing if
  testing is conducted by an outside laboratory
• FSC Third Party Phone Number is used to identify
  the phone number of the outside laboratory if one
  is used.
• FSC Test Date is used to report the date of RIP
  testing for the cigarette.
• Click the eSignature button

• Review the information you submitted at the
  bottom of the page
• If you need to change the answer to any questions
  use the Questions link on the left.
• If you are ready to submit the record click the
  Submit button

Add a Cigarette Style

• If you have another cigarette style to add click
  Add Style
• Repeat until all styles are entered.
• When you have finished Click the Done button,
  or chose Logout from the Menu on the left.

Brand Family Home Page

• This home page will display a record of the Brand
  Family registration and all cigarette styles
  assigned to that Brand Family. This is the page
  you will start on during subsequent logins for
  the Brand Family.
• After the registrations have been approved, this
  page will be updated to include the Permanent ID
  number assigned to the Brand or the Style, the
  current status of the registration, the original
  date and the expiration date of the registration.
