DOCUMENTING ACCIDENTS, INCIDENTS AND NEAR MISSES

• Documents may include
• Workplace instructions for administering first
  aid. These might include reference to equipment,
  personal protection equipment, what to do before
  giving aid, what to do, what to avoid or never
  do what to do after administering aid.
• A first aid report, which would include full
  details about situation, time, place, who
  involved, witnesses, injury, first aid treatment,
  the name of the first aider
• An accident, incident, hazard or near miss
  investigation report for including the names of
  those involved, details of the accident,
  incident, hazard or near miss (time, location,
  incident, injury, treatment, outcome), the
  probable cause and preventative actions
  (training, rehabilitation.
• It is important that all adults involved sign all
  documents.
