11MaxElect Pre-Existing Condition Limitation
- Pre-Existing Condition
- A condition for which
- The Insured received medical treatment,
consultation, care or services including
diagnostic measures, or took prescribed drugs or
medicines within 12 months prior to the Effective
Date of Coverage or the Effective Date of a
change in Coverage or reinstatement or - The Insured had symptoms that would have caused
an ordinarily prudent person to consult a health
care provider in the 12 months immediately prior
to the Effective Date of Coverage or the
Effective Date of a change in Coverage or
reinstatement. - For any additional benefit or change on a
subsequent application, a disease, mental
disorder or physical condition will no longer be
considered a Pre-Existing Condition if it causes
disability that begins after the change in policy
coverage has been in force for a period of 12
consecutive months. - For Producer Use Only. Not For Use with The

12MaxElect Pre-Existing Condition Limitation
- 12/12 Pre-Existing Condition (PECL) is included
in the base policy. We have endorsements that
allow a 6/12 and 3/12 PECL, and an endorsement
that removes the PECL from the policy. - Full flexibility in plan design, except in the
following scenarios - Required for voluntary cases without underlying
Group LTD - To consider offers for favorable voluntary cases
with less than 75 eligible employees (50) - An option to continue offer at renewal that might
be discontinued - For Producer Use Only. Not For Use with The

14MaxElect Pre-Existing Condition Limitation
- It is important to note that if a claim is
ineligible due to the pre-existing condition
limitation meaning that the date of disability
began within 12 months of the Effective Date and
the claim is denied the insured will NOT start
to collect a benefit on the 13th month following
the Effective Date. - The Claim may be considered only if the date of
disability begins AFTER the first 12 months
following the Effective Date. - For Producer Use Only. Not For Use with The

15MaxElect Pre-Existing Condition Limitation
- For GSI Coverage Only
- The Pre-Existing Condition will apply as defined.
- For GSI Coverage with Fully Underwritten Coverage
- If the fully underwritten coverage is issued
standard, even if there is an admitted medical
history and we decide to still issue standard,
the pre-existing condition limitation will not
apply to either the GSI or fully underwritten
amount of insurance. - If the fully underwritten coverage is rated
substandard, ridered, declined, or there was an
unadmitted medical history and we issued
standard, the pre-existing condition limitation
applies to the GSI amount of insurance. - Lets look at a few examples.

19 Risk of Disability
- Pre-Existing Condition Medical consultation and
prescribed medication within 12 months prior to
the Effective Date for Hypertension. - Disability Stroke (Cerebral Vascular Accident
CVA) within 12 months after Effective Date of
coverage. - Typical Result Covered Disorder. While the
Pre-Existing Condition places the individual at a
higher risk for the disabling disorder, medical
science has not documented a substantially direct
causal relationship between the two conditions. - MassMutual believes that every claim must be
analyzed individually with due regard given to
the particular facts, the provisions of the
specific policy at issue, and state laws
applicable to each situation.

- Pre-Existing Condition Medical consultation and
prescribed medication within 12 months prior to
the Effective Date for Diabetes. - Disability Visual Disorder (Diabetic
Retinopathy) within 12 months after Effective
Date of coverage. - Typical Result Disorder Not Covered. Diabetes
has well established long-term complications for
which medical science has documented a
substantially direct causal relationship between
the two conditions. - MassMutual believes that every claim must be
analyzed individually with due regard given to
the particular facts, the provisions of the
