    into the report adds no value.
  • Record the specific samples and results that were
    reviewed during the audit.
  • Process measures and outputs should be sampled,
    reviewed and recorded for every audit.
  • Audit trails may or may not take you into the
    other parts of the process (inputs, who, how,
    what, etc.)
  • Only complete the remaining sections that were
    reviewed. It is a sampling of the process. (i.e.
    if you did not review competencies then do not
    enter anything in this box)
