232/2/05 Platinum Jet at Teterboro. Near max
weight C-G substantially forward (no weight
balance), inadequate trim. Could not rotate on
6,000-foot runway. Overran at 160 knots, through
turf fence, across major road into building.
All 15 occupants survived 13 serious. Untrained
flight hostess did not know how to open the
door, then left the scene.
Part 135 operator (Darby) contracted flight to
Platinum (non-135 operator) under a certificate
management agreement. Darby exercised no
operational control and neither company followed
rigorous procedures.
24Pinnacle Airlines Part 91 positioning flight,
10/14/04. Two pilots new to RJ lost control at
41,000. When ATC asked about FL410, crew said we
decided to have a little fun and come on up
here. Then reported 1 engine out asked for
nearest airport. At 13,000 reported second engine
out. (In fact both failed at FL410) remained in
vertical speed mode mishandled re-start.
Issues professionalism crew pairing training
poor understanding of RJ service ceiling .
