Withholding
When you certify that you are claiming exemption
from withholding, you are certifying the
following
  • I claim exemption from withholding for 2009, and
    I certify that I meet both of the following
    conditions for exemption.
  • Last year I had a right to a refund of all
    federal income tax withheld
  • because I had no tax liability AND
  • This year I expect a refund of all federal
    income tax withheld
  • because I expect to have no tax liability.

If you do not meet both of these conditions for
the entire year, you cannot claim EXEMPT at any
time during the year.

UNMH Online You can find a blank Form W-4 (2009)
from the hospitals Intranet. Click on HR,
Compensation, Forms, 2009 W4 Form

Deliver W-4 forms to the Human Resources
Compensation department at 1650 University Blvd.
NE, Suite 200 or to the HR Satellite Office in
Administration at UNMH. If you have further
