Patient Passports This Patient Passport has
been introduced to help improve your journey
through the Health Service and improve
communication for infection prevention and
control between different healthcare settings
(e.g. X-ray, GP, District Nurse visits, Carers
etc). When visiting this Hospital or any other
healthcare setting please show your Patient
Passport. This will help staff involved in your
care ensure that you receive appropriate
