Podiatrist Footwear, Feet and Gait Assessment,
Gait and Vestibular Stepping Test

- Assessment results fed back to Client and CARE
PLAN developed, which may include - Lifestyle factors eg, increased fluid intake and
regular exercise - Referral to internal Sunbury Community Health
Centre services and programs - Home safety recommendations
- Letter to LMO with summary of assessment results
and recommendations for follow-up by LMO, which
may include - A review of Calcium and Vitamin D status
supplementation - Optometry review
- Specialist Clinics follow-up
