Information Sharing (IS) Index
  • A tool to help Practitioners to accurately
    identify
  • the Child or Young Person before them
  • if they are receiving universal services
  • which other Practitioners are (or have been)
    involved with the child
  • that another Practitioner has information to
    share, has undertaken an assessment or is taking
    action with regard to the Child
  • The requirements and solution structures are
    being shaped by
