• Description This form is used to file a claim of
  discrimination in the enjoyment of facilities and
  services by any place of public accommodation,
  such as a business, recreation, lodging,
  entertainment, or transportation facility that is
  open to the public in Illinois.
• Illinois Department of Human Rights, Housing/Real
  Estate Transactions Complainant Information Sheet
• Description This form is used to file a claim of
  discrimination in the sale or rental of
  residential or commercial property in Illinois.
• Idaho Nez Perce Tribal Court, Civil Complaint
• Description These forms are used to file a civil
  complaint with the Nez Perce Tribal Court in
  Utah. These forms and instructions will be
  combined to make one HotDocs template with one
