  directe of uitgestelde cholecystectomie?

• Niveau 1a Directe cholecystectomie leidt tot een
  sneller herstel met een kortere opnameduur
• geen verschil in complicaties en conversies
• In 25 van initieel conservatief behandelde
  patiënten volgt alsnog eerder dan gepland
  cholecystectomie
• goedkoper
• QOL!

• Dekking tegen enterale gram-negatieve bacilli
• Combinatie extended-spectrum penicilline zoals
  ampicillin of piperacillin met een aminoglycoside
  is OK
• Cephalosporines, carbapenems (imipenem or
  meropenem), en fluoroquinolonen ook goed
• Anaeroben komen weinig frequent in gal voor bij
  cholangitis, maar toevoeging van metronidazol aan
  noncarbapenem regimens kan zinvol zijn
