Textual Summary Errors

• PURPOSE To report eye exam errors
• Patients may report either one or more errors
  that occur during a visit
• When an error is reported, it is reviewed by a
  doctor
• Errors are classified into unique type IDs

• Each patient goes through at least one process
  step
• Patients are entered into the database whether or
  not they show up for an appointment
• Patients report errors back to the clinic

• Each process consists of at least one process
  step
• A process step may utilize equipment and
  supplies
• A patient with perfect vision and no
  abnormalities is not assigned to an eye condition

• A process may result in a diagnosis of one or
  more eye conditions
• Employees (Doctors and Clinicians) participate in
  the process steps
• Errors occur during process steps

  doctors and student clinicians
• A doctor cannot be a student and vice versa
• Each student is responsible for one process step
• Doctors review patients after each step

• Doctors advise and teach classes
• Doctors help students and rectify errors
