Whats New

• Removed Class, Facet, Stream headers and their
  semantics. PUBLISH now behaves much like OPTIONS
• Added 494 Out of Sync response code
• Added set of requirements for the body of a
  PUBLISH request (and showed how these are
  satisfied by CPIM-PIDF)
