SA Health Policy Falls and Fall Injury Prevention and Management Operational Guidelines for Health Units aim to HCSCC Charter
1. ACCESS Right to access health community services
ensure that all health services provide a safe environment through service design, planning regular review/audit. recognise that systematic proactive risk identification management is required in order to reduce risk of falling sustaining harm from falls for consumers in the health service environment. increase the awareness of all staff, patients and the public of the importance of being proactive in the prevention of falls, and the risk to consumer safety when this is not achieved. ensure that falls are included in the incident reporting and management system. 2. SAFETY Right to be safe and free from abuse
ensure a uniform approach to falls and falls injury prevention that is in accordance with the national guidelines, consistent across services and embedded in practice. improve service by being proactive in the prevention and management of falls, particularly among older people. understand that valuable lessons can be learnt from both actual and near miss incidents promote best practice in fall and injury preventionidentifying the individuals fall and fall injury risk factors implementing strategies to address these, that are resourced adequately, monitored/reviewed regularly. highlight the need for staff to have access to regular education programs, policies, procedures etc develop a suite of indicators relating to falls/fall injury that will be used to monitor trends planning. 3. QUALITY Right to quality service

ensure that consumers/carers are made aware of the outcome of assessments participate where possible in the planning of interventions provision of written and verbal support and care coordination to consumers / carers. highlight that engaging older people is an integral part of preventing falls and minimising harm from falls. ensure that consumers are encouraged to partner with clinical staff in the development of their falls prevention plan. 5. INFORMATION Right to be informed
