Urban Disincentives
  • A stick is cheaper than a carrot
  • Does preventing someone from going to the city
    force them to go to the country?
  • If they go to the country will they stay?
  • 6 jurisdictions restrict physicians from setting
    up in overserviced urban centers

Incentives - Natural limits
  • Established city docs are not likely to move and
    may have difficulty coping if they do
  • Pool of candidates for any rural position tend to
    be either existing rural doctors or recently
    trained physicians
  • Restrictive training policy limits local pool
  • Restrictive immigration policy results in net
    exodus of doctors from Canada
