LOSS PREVENTION AUDIT

• AGENCIES THAT DEVELOP, IMPLEMENT AND ADHERE TO A
  COMPREHENSIVE SAFETY AND LOSS PREVENTION PROGRAM
  WILL BE GIVEN A CREDIT TO BE APPLIED TO THE
  AGENCYS ANNUAL SELF-INSURED PREMIUM PER LINE OF
  INSURANCE COVERAGE EQUAL TO FIVE PERCENT OF THE
  AGENCYS TOTAL ANNUAL SELF-INSURED PREMIUM.
  COVERAGE FOR MEDICAL MALPRACTICE AND ROAD HAZARDS
  SHALL BE EXCLUDED.

  CERTIFICATION AFTER UNDERGOING A LOSS PREVENTION

  PREMIUM PAID PER LINE OF INSURANCE. COVERAGE FOR
