• Read INSTRUCTIONS for THE SUBMITTER before
  submitting the CONFESSIONS
• Type your confessions in Forms A (Confession
  Submission), B (Confession Discussion), C
  (Confession Follow Up) or D (Averted Confession)
• For enhanced anonymity, just send print function
  after completing the confession-filled in the
  correct form (A, B, C or D) and DO NOT SAVE to
  your computer
• IT DOES NOT REPLACE QA or MM
• If mistakenly there will be any identifiers of
  patients or operators, those forms will be
  cleaned (with white cleaning liquid) before
  review
• If mistakenly MM or QA related case is
  submitted as Confession, it will ONLY be
  discussed in QA but NOT SHARED Electronically
• Do not write on the form as your handwriting may
  be recognizable
• Place Completed Forms in Confessional Box at your
  worksite Try to do this without anyone noticing
  it so as to maintain your anonymity
• Submitted confessions will be collected, sorted,
  reviewed and shared for education monthly by our
  educators team of Dr X, Dr Y, Dr Z and Ms A.
