Purpose
  • The purpose is to complete a health and safety
    management system audit of (insert
    faculty/school/workplace) against the QUT
    National Health and Safety Audit Tool and
    Queenslands Performance Criteria and Guidelines
    for Workplace Health and Safety Reports for
    Self-Insurers

National Audit Tool Elements
  • Audits of management systems are undertaken by
    auditors using the following five major headings
    as outlined in the QUT National Health and
    Safety Audit Tool
  • Health and Safety Policy
  • Planning
  • Implementation
  • Measurement and Evaluation
  • Management Review

Performance Benchmark
  • Elements 1, 2, 4 and 5 are addressed by yes / no
    answer format, whilst in Element 3, a Level of
    Achievement is required

Weightings
  • Each topic under Element 3 is given a weighting
    ranging from 3-12, and a score is calculated by
    multiplying the level achieved by the weighting
    figure.
  • A minimum achievement of 70 of the available
    score is required in order to meet a minimum
    acceptable standard.

Audit Objectives
  • There are two objectives
  • To ascertain compliance with the requirement of
    Workplace Health and Safety Queenslands
    performance benchmark and
  • To provide QUT with a meaningful report.

The Audit Process
  • There are three phases of the audit process
  • Document review
  • Management, staff (professional and academic) and
    student interviews and
  • Workplace inspections.

Example Workplace Inspections
  • Hazard
  • Chemical decanted into unlabelled spray
    container.
  • Management System Failure
  • The workplace inspection program may not be
    detecting the issue.
  • The inspection checklist may not clearly identify
    a section to check labelling of containers.
  • Staff conducting the inspection may not know to
    look for unlabelled containers.
  • Managers and employees may not be aware of health
    and safety requirements for unlabelled containers.
