Fire Module
  • Required for Incident Types 111, 112, 120-143,
    160-173
  • Optional Wildland Fire Module can be used instead
    for incident types 140-143, 160, 170-173, 631,
    and 632
  • Information on particular types of fires or items
    involved in the fire.

Structure Fire Module
  • Required for all Structure Fires.
  • A structure is an assembly of materials forming a
    construction for occupancy or use to serve a
    specific purpose.
  • Information on different factors or items
    involved in the building fire.

Civilian Fire Casualty Module
  • Completed only for civilian casualties resulting
    from a fire.
  • A fire casualty is a person who is injured or
    killed as a result of a fire, and includes
    injuries or deaths from natural or accidental
    causes sustained while involved in the activities
    of fire control, attempting rescue, or escaping
    from the dangers of the fire.

Fire Service Casualty Module
  • Report all injuries, deaths, or exposures to fire
    service personnel that occur in conjunction with
    any incident response.
  • Fire Service Personnel Exposures are reported in
    this module also
  • Exposure occurs when fire service personnel come
    in contact with a toxic substance or harmful
    physical agent through any route of entry into
    the body (e.g., inhalation, ingestion, skin
    absorption, or direct contact). Exposures can be
    reported regardless of the presence of clinical
    signs and symptoms.

EMS Module
  • EMS Module is used gather basic data as it
    relates to the provision of emergency medical
    care to the community. It is may be used by both
    responding EMS unit(s) and fire suppression
    unit(s) that provide emergency medical services.

Hazardous Materials Module
  • A reportable HazMat call occurs when
  • Specialized HazMat resources were dispatched or
    used or should have been dispatched or used, for
    assessing, mitigating, or managing the situation.
  • Releases or spills of hazardous materials that
    exceed 55 gallons.

Wildland Fire Module
  • Purpose of the Wildland Fire Module is to
    document REPORTABLE wildland fires.
  • Prescribed Fires are also REPORTABLE wildland
    fires
  • Permits wildland fires to be profiled in-depth
    for resource allocation, incident management, and
    fire impact analysis.

Arson/Investigation Module
  • Arson Module may be used whenever the Cause of
    Ignition is intentional, or under
    investigation without any distinction made as to
    whether or not a crime has occurred, or a
    determination of criminal intent.
  • May be used to document juvenile-set fires.
  • Investigation module is now available for
    non-incident related investigation activities.
