WHAT HAPPENED NMMATERIALS FALL 150 FEET FROM
DERRICK CLIMBER ASSIST. During a post
rig/derrick stand-up inspection, the two climber
assist counter weight anchor lines were found to
be tangled, causing one of the counter weights to
slightly invert. As attempts were made to
untangle the lines, the contents of the counter
weight spilled and fell approximately one hundred
and fifty feet to the ground. The counter weight
contents consisted of one large rock and one
four-inch anchor pin weighting approximately 25
lbs.

raising derrick. - JSA and work plan to untangle lines did identify
the inverted counter weight and the associated
hazard of the counterweight contents. - Poor design of counter weight (open top and no
means to view direction of travel). - Non-engineered and improperly secured materials
used to provide weight.
