a an individual, a population, companys insured
employees and dependants, - HDQ is based on the Health and Disease Model
i.e., one that cares for health with the same
intensity it cares for disease. - It takes most people 10 minutes to complete.
- Primary care or Family physicians could be
rewarded accordingly.
See Rewards for usage by Milt Seffert, M.D.

creative between the patient and clinician - HDQ tool facilitates interdependency for both
individuals and purchasers through a common
language - Patient Choice an individual is more informed
and better able to can take action on the results - Focus is on both overall holistic health and

11Harmony with Health and Life Management Skills
- Dr. Milton Seiffert, M.D.
- The maturation of this technology dates back to
Alcoholics Anonymous in the 1950s the Eagle
Medical practice began using this approach in
1972. The concepts of the life management skills
program are simple, understandable and
appropriate even for children as young as eight
years of age. - In short, this programs most important feature
is its documented capacity to change (health)
behavior. It has shortened in-patient hospital
length of stay, reduced domestic violence,
improved marriages, fostered (the) practice of
staff interdependence, increased seat belt usage
also of great significance is the absence of
suicides of patients in the program since its
inception in 1972. Over and over, this technology
has helped people with chronic disease to modify
their behavior, as well as those with compulsive
disorders. The Eagle Medical practice has found
that this approach can be used to treat the
emotional component of almost any medical
practice problem, e.g., medical illness, surgical
illness, chronic pain, or autoimmune disorders.
12The HealthChord Movement Phase 2
- There are various views of the U.S. healthcare
system - AARP, Mayo Clinic, the Institute of Medicine just
to name a few - All have a part of the truth but provide a
picture that is incomplete. All say change is THE
priority - Articles found at www.healthchord.info
- Click on Primary Care
13The HealthChord Movement
- Crossing the Quality Chasm to better healthcare
from the Institute of Medicine focuses broadly on
how the U.S. healthcare system can be reinvented
using a Primary care approach.
- Even though the healthcare system as a whole is
in huge disrepair, we also believe is is possible
to create a safe and effective system. - In this we agree with the Institute of Medicine,
Mayo and AARP
However, it will require proven methodologies to
speed the transformation of the current chaos
into a system that works well for all.
14The HealthChord Movement
- The system and process must begin at the
local level and must
involve many parties,
both groups and individuals. - Patient and Doctor having open and honest,
quality dialogues that get quality results. - HealthChord would unite the principal
stakeholders, I.e. doctors, patients and
purchasers, as each of these would benefit from
higher health status
15Deming on the System
- Edwards Deming on results
- - the system accounts for 80
- - individuals account for 20
- When problems in production occur, it is useful
to examine the system first.
16Primary Care Costs Less
- Worldwide there are disparities and inequities
in healthcare
(including in the United States). - The biggest factor for disparities is NOT poor
and developing
countries but rather the system -
- Numerous studies show that a Primary Care focused
healthcare system produces better health outcomes
and costs 30 less - Primary Care is anchored in a Partnership that
believes - - good healthcare outcomes requires a good
clinician/patient relationship - - A good relationship requires mutual
negotiations and, - - Successful negotiations require a common
language - See
Negotiation Medicine
17Accessible
- It is organized to provide medical care around
the clock, seven days of the week. - It has an Accessible governing body - the
Patient Advisory Council (PAC), active since
1974. The PAC is voluntary for both practice
staff and patient group. Its purpose is to -
- resolve complaints (Services Improvement)
- develop health policy
create community education
forums and
provide
patient-to-patient services. - be helpful and never harmful
18Accountable
Practice Accountability - By virtue of its
governing body, the patient advisory council
(PAC) makes resolving patient concerns or
complaints it most important act of
accountability Patients Accountability -
promoting greater patient responsibility for
health through self-management concepts and
techniques that always stay within the context of
a strong doctor/patient partnership.
- Patients gain responsibility through exercising
individual choices. - A greater sense of control over health, promotes
self-initiated preventative care, more optimism
about the efficacy of early treatment, and
greater actual and perceived health. - The belief that they can positively affect their
own health encourages people to behave in more
positive ways with gratifying health results. - The doctors get patients who are better partners
and this improves the outcomes.
19Comprehensive and Continuing
- Integrates the care of patients biomedical and
psychosocial needs and
avoids the
fragmentation of mind-body dualism. - Treats mental, emotional, social and spiritual
health right
within the practice setting, reducing
the need for
additional consults. - Employs a health educator with the title, Health
Coordinator. Note Patients rate the role of the
health coordinator highly - Helps patients develop effective life management
skills especially in the area of stress
management. - HDQ results are often used as a first line to
treatment, reducing or avoiding the need for more
expensive technology or mood-alerting medications.
20Coordinated and Customized
Once Eagle Medical understands the patients
strengths and needs, clinic caregivers rely on
teamwork and interdependence to meet patient
goals and to provide individualized care.
- Customized care via the Health and Disease
Questionnaire (HDQ), which Measures both
strengths and risks of patients in the areas of
physical, mental, social, emotional, and
spiritual health. - Confidential HIPPA sanctioned and Secured
Questionnaires are completed online. - Results are received within two minutes, real
time, which allows the doctor to
encourage the patient to begin
positive health behavior change.
21Coordinated and Customized
- Promotes an interdependent team approach between
and among providers. - Allows more providers to participate effectively
in safe and effective resolution of complex
patient problems. - The Primary Care physician serves as the team
evaluator, helping the team hone diagnoses,
re-diagnose, and seek new treatments as well as
greater continuity of care. - This team approach allows for early intervention
and treatment of patients at a level in the
system that is most personal, most appropriate,
and least costly.
