1Broadspire TelePlus Claim Capture

2The first step is to select the PEO that the
claim is being reported for. Users will only
have access to the PEO they are authorized for.
The second step is for the user to enter their
Name, Phone Number, Date of Loss and Time of
Loss. As information is entered, the user must
TAB through each field.
3A unique reference number is assigned for each
loss reported.
WELCOME screen will provide assistance with the
application.
4Messages appear throughout the call flow to
assist a user.
5The worksites related to the PEO will display
for the particular state being entered.
Either Highlight and Select one of the worksites
or if the worksite is not displayed, select Quit
to continue with the claim intake.
6Cedar Hill account Specific information is
gathered on this screen
7The PEO information will prefill based on where
the claim is being reported
Each screen has a help button that will display a
HELP page. The HELP page provides assistance for
required entry.
8The loss location will prefill with the worksite
address that was selected. If the loss occurred
at a different address, the information can be
updated.
A zip code search is available that will
populate the city and state fields.
9NCCI Job Classification search is available or
manual entry is allowed.
Additional Cedar Hill information is gathered
throughout the claim intake.
10Injury coding is gathered based on standard
industry NCCI codes.
Fields will enable or disable. The date fields
enable when the Loss Time field is equal to YES.
11Data entry assistance is displayed.
12Information regarding either the hospital or
physician can be entered.
13Witness information is displayed.
14Data entry assistance is displayed.
15Additional comments can be sent to the claim
handling office..
A copy of the claim acknowledgement will be sent
via fax based on the number entered.
16Instructions for completing your loss are
provided.
Information regarding the Broadspire Claim
Service Center will display at the end of the
call.
17The Broadspire reference number is displayed
when the loss is submitted.
