Use either text or a graphic for your survey
title. Perhaps you prefer to have your company
logo at the top of your survey.
Select the name that will represent your survey
to the ASCP internally. May only contain letters
and numbers. No spaces or special characters.

Pick a name that you will remember. Must be 10
or fewer characters

Select Question Type-Radio Button
I want my first question to use radio buttons.
  • You must select the format of your question.
  • Radio buttons and Single Selection Drop Boxes
    are most appropriate when the respondent may
    only choose one response.
  • Check Boxes and Multiple Selection Drop Boxes
    are most appropriate when the respondent is
    meant to check all that apply
  • Numeric Responses are for actual numeric
    responses
  • Short Text Responses are for simple text
    responses up to 50 characters in length.
  • Long Text Responses are for paragraph type
    responses up to 4000 characters.

We are going to skip the steps to add the rest
of the questions and begin adding some
Instructions and Comments.
Click here to begin adding instructions or
comments. Note This can be done at any point
in the Survey Creation process, you do not need
to wait until the end.

Select a font color for just this instruction or
comment.
Select a font type for just this instruction or

Update/Edit Survey Layout
You can add as many instructions and/or comments
as you like. They may be of any length that you
choose. We are going to skip ahead and add a few
more comments to complete the survey.
Now we are going to move on to using the Editable
Survey View to edit our survey. First, we are
going to delete an unwanted question.

Delete Questions
We are going to delete questions 1 5 by
checking the Delete Question boxes and then
Update New Changes

Rearrange Question Order
You can see that the questions we selected, I
would enjoy having a job or career that required
extensive use of a foreign language. and How
old are you?, are now gone and that the survey
has been renumbered.
Now, we are going rearrange the survey. I want
question 1 to be question 16, and question 2
to be question 1. To do this, I simply change
the number in the Order Number box for the
corresponding questions. All of the other
questions should be automatically
renumbered. Dont forget to update changes!!

Rearrange Question Order
Editing can be done in multiple steps or all in
one step. Be aware though, that if you perform
multiple deletions and renumbering in the same
step that you need to take into account the
automatic renumbering of items by the editing
process. Check the final order of your questions
carefully. For example If you want to move the
question What is your gender? to be in the
2nd position in your survey and you enter 2 in
the Order Number box, and in the same move,
you want to move the comment Personal
Information from the 20th position to the 1st
position, when you finish, What is your
gender? will actually be in the 3rd position.
I am going to skip ahead again, and renumber the
comments in my survey, just like I did to the
earlier questions. Now, we are going to preview
what your survey will actually look like in the
end.

Complete Final Survey Settings
This is the style selection page for creating
your survey.
Like the pages for title and instructions, you
first select the font type and color for the body
of the survey
Next, you select the survey format, either
traditional (solid background with a border
around it) or boxed (alternating lines of colors)
If you want to require users to enter a username
and password to access your survey, you can
upload a comma delimited file with one username
and password per line
Next, select your background and border colors.
If you would like users forwarded to a specific
web page after they complete your survey, you can
enter that URL in the next box.

  • Review you final survey and either
  • Return to Editing Mode
  • Or
  • Complete the Survey Process
