How we have been working on secondary
prevention in our area

• Reduction of Atherosclerotic Disease in
  Shropshire (RADISH) project
• Patient satisfaction questionnaire
• Extension of use of patient hand held record to
  include PVD, CVA, heart failure and possibly
  diabetes mellitus
• Cardiac Network Health Equity Audit
• CHI recommendations

Our successes to date

• Results of the patient questionnaire showed high
  levels of satisfaction with the book among
  patients/carers
• Practice nurse responses to the questionnaire
  have informed the direction of redesign for the
  RADISH book
• Data showing improvement in drug use (aspirin,
  statins)

• Data collection
• Getting consistent data from across the patch
  (will QMAS help?)
• Engagement
• how do we tackle those who dont engage with us
  and who might need help?
• Continuing to work to the CHD NSF

• Work with those who are keen to work with you
• The wholehearted support of clinicians and
  managers is required for any initiative to work
• Each StHA, PCT GP practice has its own way of
  working
• What works in one area may not necessarily work
  in another
