Admission Process

• Nursing will enter the Home Medications into the
  computer system
• A document will be printed out for the provider
• The provider updates the document by hand
• Each page of the document needs to be signed by
  the provider

Discharge Process

• 2 documents are printed
• One for HOME medications
• One for ACTIVE INPATIENT medications
• The provider updates the documents by hand
• New orders are added to a blank order form
• Each page of the document must be signed by the
  provider
• The new discharge list becomes the initial home
  medication list on next admission

Dont Forget!

• Each page must be signed by the provider with
  date and time
• Address each medication one line item at a time
• To continue Home Medications after discharge
• select Continue in the HOME medications section
  
• Discontinue the corresponding ACTIVE INPATIENT
  medication
• Use CHF Core Measure Discharge Instructions
