OUR AIM AND OBJECTIVES

  improve the health and well-being for all.

• ADMISSION AVOIDANCE
• For example
• Palliative Care
• Nationally and locally there is evidence that
  patients are admitted to hospital to die when
  this might not be the most suitable environment
  for them. There are opportunities to commission
  services to enable them to remain at home.
• Managing at risk Patients
• Working with practices to implement a risk
  stratification a tool to identify patients who
  could potentially be admitted as emergencies but
  who may, with effective management, remain at
  home. Commissioning services which support the
  delivery of their management.

• DIABETES
• Implementation of the model of care during
  2009/10 the implementation of this model of care
  will require a programme of work on engagement,
  commitment and training for member practices.
• Focus on obesity management obesity is a
  potential health risk for Somerset. It is a risk
  factor for many long-term conditions and there
  are opportunities to commission a range of
  innovative services.

• EMOTIONAL HEALTH AND WELL BEING
• Delivering faster access to high quality and
  timely, psychological therapies
• to adults with mild to moderate common mental
  health problems.
• Reducing the risk factors for depression and
  dementia, whilst supporting
• and promoting positive mental health to improve
  life satisfaction,
• particularly for older people
