  • Employees log onto www.theEAP.com and complete
    the health assessment
  • Upon completion each member
  • Receives a complete wellness status summary
  • Is given written recommendations on how to
    improve overall health status
  • Is directed to his or her ESI wellness counselor

Wellness Counseling
  • After completing the Health Risk Appraisal,
    members call to schedule telephone counseling
    session with a designated wellness counselor
  • Counselor assesses the member needs, provides
    support and counseling and refers him or her to
    the appropriate covered benefits and services
  • When appropriate, referrals are also made to
    community based programs
