- Employees log onto www.theEAP.com and complete
the health assessment - Upon completion each member
- Receives a complete wellness status summary
- Is given written recommendations on how to
improve overall health status - Is directed to his or her ESI wellness counselor
9Wellness Counseling
- After completing the Health Risk Appraisal,
members call to schedule telephone counseling
session with a designated wellness counselor - Counselor assesses the member needs, provides
support and counseling and refers him or her to
the appropriate covered benefits and services - When appropriate, referrals are also made to
community based programs
