    specialist will assist you by telephone 24 hours
    a day, 7 days a week, 365 days a year

of
  • An on-line Medicaid application
  • Streamlined processing of applications for
    certain Medicaid waivers
  • An on-line form that you can fill out to tell you
    if you might be eligible for certain supports

No More Agency Hopping
  • Local agencies are currently joining together in
    6 No Wrong Door implementation sites to improve
    your experience in obtaining and providing long
    term supports
  • Plans to expand to 10 sites by September 2008
  • Goal is a network of No Wrong Door resource
    providers statewide by 2010.
  • The vision
  • Create a comprehensive network of aging and
    disability service providers that collaborate and
    plan together
  • Electronic sharing of information with your
    permission to avoid your having to fill out
    multiple applications for supports at multiple
    agencies

    for moving to the community if that is your
