  • 1 Fall Mid August
  • 2 Winter Early October
  • 3 Spring Late February
  • Date are set based on the school calendar each

    Golf, Cross Country, Cheerleading
  • 2 Winter Basketball, Bowling, Wrestling,

School Visit Checklist
  • Posted on MIAA Website
  • 1 CPR/First Aid current for all coaches
  • All coaches supervising activities MUST be
    CPR/First Aid certified
  • 2 TSSAA Eligibility Report
  • Reports must be fax to MIAA office
  • Copy to be on file with AD

  • Physical by FNP, MD, or PA, with signature,
    date, address and phone number
  • Completed medical/contact information
  • Consent for participation
  • Parents Signature
  • These item are required before an athlete may
    begin participation.

  • 4 Records stored in a secure location
  • 5 Equipment properly kept and reconditioned
  • Proof of reconditioned equipment, i.e. football

    mentioned paperwork

  • Be prepared for the visit
  • Your assigned athletic trainer will contact you
    to schedule your review, if you have not already
    scheduled your visit
  • If you are not ready, reschedule

  • Have all documents on hand prior to the visit
  • Review all files and note any paperwork that is
    missing

  • Prepare a separate folder, notebook or file for
    each sport
  • In each folder, place the following info
  • 1 Copy of CPR cards for the entire coaching
    staff
  • 2 Roster
  • 3 Athletic Health Records in alphabetical
    order
  • 4 Include a list of items missing

  • Copies have at least 3 copies of the athletic
    health record
  • 1 Master (original) copy to be kept with
    Athletic Director
  • 2 Copy to be placed in site visit file
  • 3 Copy to remain with coaching staff
  • This copy is to be with the coach at all
    times, practice, games, travel

CPR/First Aid Certification
  • Please coordinate with your coaching staff to
    make sure all members are CPR certified. It is
    an MIAA regulation the all coaches, faculty and
    non faculty, are CPR certified. These courses
    are paid for by the MIAA. If any coaches desire
    to become certified by any other means, they are
    responsible for the cost.

Game/Practice Equipment
  • The following items are required at all athletic
    events, practices, and games
  • Fully stocked first aid kit (Minimal supply list
    is located on our web site)
  • Injury ice with bags
  • Drinking water, bottles, coolers (Have enough to
    last the entire session)
  • Athletic health record with coach

Athletic Insurance
  • Required for all student athletes
  • Payments are due prior to the first regular
    season competition
  • If payments are late refer to original TSSAA
    eligibility report for squad size, not squad size
    at time of payment

  • Please send a separate check for each sport,
    included an eligibility report with each check
  • If paying for multiple sports on one check,
    please include a breakdown of what sports the
    payment covers
  • Fines may be assessed for late payments
