4Fall Grant Application Deadline
- Wednesday, September 7 at 400 p.m.
- Applications will be accepted before September 7.
- Applications will be accepted from 9 a.m. 400
p.m. on September 7 at the Baptist Healing Trust
office.
5Eligibility Criteria
- The Program Committee sets the eligibility
criteria for grants. - This includes type of non-profits to be funded,
geographic area to be funded, areas of interest
and exclusionary criteria. - Areas of interest are funding areas that the
board has identified based on community needs to
direct their grant investments.
6Eligibility Criteria
- Charities that impact individual, group or
community health and specifically target groups
or populations that lack access to healthcare. - Charities whose work is targeted toward the
Middle Tennessee area. - Organizations must be 501(c)(3)
7BHT is unlikely to fund
- Grants directed toward the operational and
administrative activities of an organization - Grants for the construction or renovation of
building facilities - Grants to individuals, fraternal organizations,
and research oriented single-disease charities - Grants to other grant-making organizations
- Grants to support partisan politics, political
lobbying, or electoral campaigns
8BHT Partnership Grants
- are made to health related non-profit
organizations to provide extra support necessary
to apply the principles of radical loving care
through funds for capacity building, leadership
training and consulting. BHT Partners receive
funds for both the direct and indirect expenses
associated with undertaking transformational
change. - BHT seeks to empower non-profit organizations to
take a more strategic approach in tackling the
challenges they face, to craft new ways of
enhancing their effectiveness, to strengthen
their staffs, and most importantly, to put into
practice radical loving care within their
organizations.
9Example of Partnership Grant
- Alive Hospice
- Received a 15,000 grant to enable Alive Hospice
to continue to advance a purposeful culture of
love through leadership development and staff
training. Hospice is tracking employee and
patient satisfaction to evidence improvement.
10How to apply for a partnership grant
- An organizational needs assessment conducted
through meeting(s) between the BHT staff and the
non-profits staff - Partnership application given to interested
organization after initial meeting - Complete or update your agency profile on
GivingMatters.com - Submit partnership application by grant deadline
on September 7th.
11Impacting Education Grant
- Impacting Education Grants are for specific
programs or courses, which impact medical and
other health professionals during their education
and training. The Trust seeks to encourage
nursing, medical and other allied health
professional schools as well as postgraduate
training programs to introduce a course or
program advocating a loving care model of
leadership and culture or a similar program
addressing spirituality and compassionate care.
12Examples of Education Grants
- The Belmont School of Nursing - 12,475 to
implement ongoing faculty and staff retreats to
reevaluate the core processes and mission of the
School and to ensure radical loving care is the
heart of their daily work with students, patients
and colleagues. - Vanderbilt Childrens Hospital - 25,000 to
create a program that trains pediatric residents
in family centered care. VCH is tracking patient
surveys to assess the success of the program.

- Contact the Trust to arrange a meeting with the
CEO and Program Officer - Impacting Education application given to
interested organization after initial meeting - Complete or update your agency profile on
GivingMatters.com - Impacting Education proposal submitted to BHT by
September 7th.
14Transom Grant Overview
- Typically 25,000 - 75,000 (minimum of 25,000)
- Typically one year term
- BHT will fund more than one year, for new
programs, on a very limited basis - Program or project based support
- Outcomes based
- Organizations can only be funded once/year

- Examples Crisis Intervention, Mental Health

Residential Treatment Services, Outreach and
intervention to high-risk populations. - Healing from Abuse, Neglect or Violence

Therapeutic Services, Survivors Services.

- Before writing this grant, if you havent done so
already, contact the Program Officer to ensure
your program fits the funding priorities! - The ABCs of writing this grant
17Transom Grant Application Guidelines
- Transom Application Cover Sheet signed by the
CEO or Executive Director of your organization.
The Cover Sheet should be no more than one page. - Executive Summary describe program or project
for support, community impacted, experience in
reaching, engaging and serving this population,
time frame, brief description of how work will be
accomplished, and collaborative partners and/or
funds. Limit your Executive Summary to one page.

- Needs Assessment - give evidence illustrating the
community need for the program and detail how the
community will benefit. Limit your Needs
Assessment to one page. - Helpful Hint Its a good idea to include local,
state and national data as evidence of the need
for your program. Waiting lists and number of
people turned away from program due to lack of
resources also help to illustrate local need.

20Program Design Explained
- Goals broad statements of the programs
intended purpose. Not usually measurable. - Example To provide treatment and emotional
support needed by low-income domestic abuse
victims in Davidson County to overcome the impact
of abuse.
21Program Design Explained
- Objectives More specific than goals. Can be
measurable in the number of people served or in
the output of the program. - Example 270 domestic violence victims will
receive, on average, 10 counseling sessions and
will reduce their symptoms associated with the
trauma of abuse and violence. - Output is the term utilized by the United Way
logic model to describe the direct product of the
program activity. For example, numbers of
participants served, counseling sessions
conducted, etc

- Related Activities a brief explanation of the
work or activities that will occur in order for
the objectives and outcomes of the program to be
met. - Example The services for domestic violence
victims will follow a crisis intervention model.
Therapists will see clients within a 24 hour
period after an initial needs assessment by an
intake coordinator. Etc
23Program Design Explained
- Expected Outcomes- Beyond the number of people
to be served or the output of the program, the
outcome statements illustrate how those people
who are served by the program will be positively
impacted. Outcomes are measurable and very
specific! - Example 210 out of 270 (or 78) of domestic
violence clients will reduce at least two of
their symptoms to the normal range.
24Program Design Explained
- Evaluation Indicators demonstrate how you
intend to measure your programs outcomes. - Example Clients will complete a standardized
test that measures their level of symptoms such
as anxiety, depression, self-esteem and
post-traumatic stress at the beginning of
treatment. Tests are repeated on a quarterly
basis and compare the client to the population in
general (the norm).
25Biographical Sketch
- If your program proposes to add a new staff
member, please include a brief biographical
sketch that includes this persons
qualifications. This does not need to be
someones resume. We are looking for what type
of experience and degree you would like for this
staff member to have. - This may add one additional page to your program
design.

- Budget for entire program including BHT
investment. - BHT wants to know exactly how their investment
would be spent utilizing the columns, amount
requested from BHT. - All sources of revenue, including the potential
BHT grant, should be included in the revenue
section.
28Budget Justification
- A detailed description of each budget line item
and how the totals per line item were reached.
Budget Justification should be no more than one
page. - Example
- Salaries Wages The Domestic Violence Therapy
Program consists of 2.3 FTE as detailed. - Shannon Watson and Nina Garcia are Clinical
Therapists at 1 FTE each and annual salaries of
31,000 each. - Lakshmi Singh, MSW, is Clinical Director at .3
FTE and a proportionate annual salary of 14,452.
29Program Sustainability
- BHT may not be able to support your program each
and every year but does not want to be
responsible for the closure of a program or the
laying off of staff. - Program sustainability is a very important part
of your proposal. - BHT wants to know that you are committed to
sustaining this program and wants to see a
detailed plan of how you will continue to
implement your program without a BHT grant.
30Program Sustainability
- This is a good place to explain what you are
doing to counteract deficits that we may see in
your audit or current financials. - For example, if your fundraising revenue has been
down over the past two years, you might explain
that due to the downturn in the economy you have
seen a decrease in corporate donations however,
you are working to increase individual donations
through targeting your mid-level donors to give
25 more this year. - Program Sustainability should be no more than one
page in length.

- BHT might contact a few of your Directors to ask
them questions about your organization. Please
indicate who are the officers on your board. - Most Recent Annual Report
- If you do not publish an annual report, please
attach a sheet of paper in its place indicating
this. Please attach instead organization and
program statistics.

- Most Recent Audit/ Audited Financial Statements
- Please include your entire audit. If you had
audit findings, please include any letters/memos
that indicate what you are doing to remedy the
situation. - If you do not conduct an audit, please contact
the program officer beforehand. You may be able
to include your most recent 990. - If your audit is on GivingMatters.com, you do
not need to include it with your proposal.

- Current Financial Statements
- Because audits are usually only available 6 to 12
months after the close of your financial year,
BHT asks for current financial statements. - Current financial statements should include a
year to date statement of revenue and
expenditures within two months of the grant
deadline. - If the grant deadline is September 7, BHT would
want to see current financials through July 31st.
- If you are on a fiscal year July 1 June 30, we
would want your 04/05 financials and your July 05
financials.

- IRS Determination Letter indicating 501(c)(3)
status. - This letter should be issued from the IRS and
contain the date of determination and your
organizations Federal Identification Number or
Employee Identification Number. - If the organization name is different on your IRS
determination letter please indicate why. - If your determination period has expired, please
let us know that you have submitted the
appropriate paperwork to continue your 501(c)(3)
status

- Grant Application Deadline September 7 at 400
PM - All grants must be either mailed to or turned
into the BHT office. Mailed proposals must be
received on or before September 7. - 4 hard copies of your proposal and 4 copies of
your attachments (in the stated order) are
required. - A copy of your proposal on a labeled disk (CD or
floppy) is required. (Please test your disk
before turning in your application.)

- We will look at your organizational information
on GivingMatters - Specifically, if your audit is on GivingMatters,
you do not need to include it with your grant
application as we will view it on the website.
