Textual Summary Errors
  • PURPOSE To report eye exam errors
  • Patients may report either one or more errors
    that occur during a visit
  • When an error is reported, it is reviewed by a
    doctor
  • Errors are classified into unique type IDs

    step
  • Patients are entered into the database whether or
    not they show up for an appointment
  • Patients report errors back to the clinic
