MEMBERSHIP REQUIREMENTS

• Full Members 25 case credits per year
• Affiliate Members 5 case credits per year
• CCOP Members 10 case credits per year
• Must have a data quality score of 80 or better

EVALUATION PROCESS

• Evaluations are done in July
• Based on the previous year
• Based on Case Accrual and Data Quality
• Audit score if performed in prior year
• Shows accrual by time period
• Shows accrual by study within time period
• Data Quality separated in categories Total
  Cases, Forms, Dosimetry, Systemic Modalities
  Surgery
• Overall Score

CASE REIMBURSEMENT

• Payments made on a monthly basis
• Case Payment is based on
• Receipt of the I1 (Key Form)
• Case Eligibility
• Payments are made to the institution
• Follow-Up and Pathology Payments are run twice a
  year in December/January June/July.

• Current Reimbursement Amounts
• 2,000 for Treatment Studies
• 1,000 for Cancer Control Studies
• 500 For Quality of Life
• 2,500 for Treatment Studies w/Quality of Life
  Component
• 50 per Follow-Up Form (F1/FF)
• Pathology- 50 (saliva), 100 (slides) 200
  (blocks) 300 (complex materials) (for example
  blood)
