1 IMMIGRATION EXAM
Kids under 14 years old have special discounted
price for the limited time Only TWO visits to our
office and the exam is completed.
In order to apply for a U.S. green card (lawful
permanent residence), you must undergo a medical
examination. The immigration medical exam must be
conducted by a physician designated by USCIS
(United States Citizenship and Immigration
Services). Oakton Health Center has authorized
physicians who will perform this exam, the
required blood draw and administer the necessary
vaccinations.

2 Form i-693 medical exam
- Generally, all applicants filing for immigrant
visas and some non-immigrant visas, refugees and
adjustments of status to that of a permanent
resident, must submit Form I-693 medical
exam completed by a designated Civil surgeon.
3Civil surgeon
- A Civil surgeon is a licensed and experienced
doctor (Physician) approved/authorized by the
USCIS to conducted immigration medical exam. - Oakton Health Center has Civil surgeon on duty
six days a week. We are always prepared and ready
to provide the service.
4 After the medical exam
- After the medical exam is complete, the Civil
surgeon will certify the results on the
appropriate forms and place them in a sealed
envelope with label on the back DO NOT OPEN
THE SEALED ENVELOPE. - Do not accept it if it is not in a sealed
envelope. USCIS will return the form to you if it
is not in an envelope or if the envelope has been
opened or altered.
