P4P 4th Anniversary Reach Impact

• Over 1,600 participating Providers have impacted
  the lives of more than 37,000 ACCESS Plus
  Enrollees.
• Over 5 million paid out in incentives to
  enrolled Providers (Program to date).
• These dollars are over and above standard fees
  paid for services as determined by OMAP
• Participating offices care for 37,000 DM patients
• of which, gt9,000 are high risk patients

• On the ACCESS Plus P4P 4th anniversary, we are
  pleased to announce program changes that are
  designed to
• Align payment opportunities even more closely
  with desired chronic condition management
  outcomes
• Compensate providers more for time spent on
  providing and coordinating appropriate clinical
  care
• Capitalize on established momentum to continue to
  improve the health of and access to care for
  ACCESS Plus Enrollees
• Reduce the volume of P4P paperwork for provider
  offices

Getting The Word Out

• Participating providers received information
  about these changes in several ways, including
• An additional letter specifically outlining
  program updates
• The updated Summary of Payment Opportunities for
  PCPs document which summarizes all the currently
  available payment opportunities
• Visits from field staff in the area who can
  discuss changes
• Reminders about program changes/updates in their
  regularly scheduled payment mailings
• Updates on the ACCESS Plus website
