2Family and Medical Leave Act of 1993 (FMLA)
- Eligibility Full-time employees one year of
service - Part-time employees one year of service of
1,250 hours or more - Notification 30 days advance notice required
(unless emergency) - Entitlement 12 weeks of unpaid,
job-protected leave to eligible
employees for family medical reasons under
Federal law - Reasons for taking leave
- Employees serious health condition
- Birth of a child, adoption or foster care
(available to both parents) - Employees spouse, child or parents serious
health condition

- Sick Leave (all employees)
- Vacation (staff non-exempt and professional)
- Flex (staff non-exempt and professional)
- Professional Staff Accrued Leave PSAL
(professional exempt contractual) - Bank points
- Sick, Vacation, PSAL accrues as long as employee
is on paid status - Sick Leave Pool
- Available when all leave time (such as
sick/vacation/bank point/flex/PSAL) has been
exhausted - Sick leave pool enrollment occurs during annual
Open Enrollment - Eligibility 10 days in reserve as of Oct.1
- To enroll, participant must donate 5 days
(assessed every year)

Personal Leave Without Pay
- Allowed for a maximum of one (1) year, subject to
departmental need - Request through the immediate supervisor
- Possible renewal on case-by-case basis
- Position Status
- Approved no guarantee to hold job
- Not approved will not hold job
5Insurance Option while on leave
- Under FMLA Employee (single coverage) health
dental coverage paid by the College (if enrolled)
for maximum of 12 weeks subject to medical review - Other benefits and dependent coverage continue
to be paid by employee - After all paid leave FMLA exhausted, all
premiums shall be paid by employee - Employee will be notified by mail about options

Disability Insurance
- If employee has disability income protection
insurance, employee may be eligible to use Short
Term Disability benefits through Assurant, Inc. - Claim forms may be requested through Benefits
main number x- 72010.
7Steps to follow for FMLA Sick Leave Pool
(contd) (30 days before leave)
- Send to
- Letitia Rackley
- HR BENEFITS Room 1114 KENDALL CAMPUS
- for medical review
- If approved, HR sends copy of approved form back
to department
- Fill out P2 form (see link on last page)
- Attach doctors note or completed DOL doctors
certification - Obtain required campus/division signatures
- Complete a Medical Release Form (see link on last
page)

Rackley when leave time has been exhausted for
Sick Leave Pool approval and /or FMLA - All extended medical leaves of 30 days and more
or requests for SLP or FMLA or complications will
be reviewed by the Colleges independent medical
consultant
