4The Plan for Transformation
- The Plan for Transformation is much more than new
- housing. Under The Plan for Transformation, whole
- neighborhoods are changing for the better.
- The City of Chicago is improving streets,
schools, parks, libraries and other
infrastructure so that these neighborhoods are
strong enough to attract a mix of incomes. - Private investors are bringing new housing,
stores and jobs to the neighborhoods.

families in public housing reveal - Low employment rates (48 unemployment)
- Low education levels (44 of adults report not
completing HS) - Low literacy rates (a sample suggests as high as
63 of adults read below the 8th grade)

Service Delivery - Strength Based Model - Emphasize a holistic
approach that builds on the positive factors each
individual or family brings to the relationship,
rather than focusing solely on what is missing. - Graduated Incentive Plans - Celebrating all
efforts and successes, while recognizing setbacks
for what they are opportunities to learn and
grow. - Specialized Group Sessions Providing a platform
for participants to raise concerns, receive
guidance and build motivation, and recognize that
ambivalence is endemic to the process of change. - Harm Reduction Model Helping participants
survive and cope with traumatic life events and
understanding that alternative coping strategies
must be developed to ensure successful outcomes.
