Definition of Haemovigilance
  • "a set of surveillance procedures from the
    collection of blood and its components to the
    follow-up of recipients, to collect and assess
    information on unexpected or undesirable effects
    resulting from the therapeutic use of labile
    blood products, and to prevent their occurrence
    or recurrence (NHO, 1999).

    surveillance procedures relating to serious
    adverse events or unexpected events or reactions
    in donors or recipients and the epidemiological
    follow-up of donors (Directive 2002/98/EC).

    maintaining complete transfusion traceability
  • Devising, Implementing, Updating
    Optimizing an effective traceability system
  • Education and Informing Clinical Staff
    regarding traceability and the need for
    compliance.
  • Regular Clinical Audit of the traceability
    system implementation of remedial action as
    required
  • Investigation Follow-up of breaches or
    deviations evidenced in the traceability
    system appropriate remedial action as
    required

Role of HVO in Notification of Serious Adverse
Events.
Devising, Implementing Updating
efficient reporting mechanisms that
facilitate capture of all suspected transfusion
reactions, adverse events incidents, errors,
inappropriate transfusion and near- miss events,
in the clinical transfusion practice area.
Continuous Education, Informing Updating of
Clinical Staff regarding established reporting
mechanisms, when they should be initiated and
the importance of total compliance. Investiga
tion, Follow-up Recording of all suspected
transfusion reactions, adverse events,
incidents, errors, inappropriate transfusion
and near miss events. Reporting of transfusion
reactions, adverse events, incidents, errors
inappropriate transfusion. Initiating remedial
action as required follow up.
In conjunction with the Blood Bank HBTC most
HVO fulfill an important role in the provision of
