  Self-Administration Medication Form
• Must be on file with the school for your son to
  be in possession of or take any medication
  (including over the counter medications such as
  Tylenol, Motrin, etc.
• Not required, but may be practical to have on
  hand for the occasional headache, or bump/bruise
  incurred during a game or at practice

• Must pass 5 classes with a minimum of 3 Cs
• 10th 12th graders
• Must pass 5 classes with a minimum of 4 Cs
• See packet for specifics on the timing of when
  eligibility is assessed

• Must be signed by both player a parent
• Violations of this code will not be tolerated
• Penalties for violations are spelled out
• Parents, I enlist your assistance in not creating
  opportunities for your son to violate these rules
