2004/2005 Highlights
  • Major accomplishments during 2004/2005 include
  • Completed consultation workshops in BC, AB, SK,
    MB, ONT (Phase 1), Maritimes (joint NB/NS/PEI)
    and NL to introduce CPSI and to identify
    priorities
  • Sponsored Halifax 4 and confirmed sponsorship
    and planning role for Halifax 5
  • Held successful patient safety communicator
    workshop in conjunction with Halifax 4
  • Establish a working group with ISMP Canada and
    CMIRPS to assess systems of reporting adverse
    events and to determine appropriate next steps
  • Established dialogue with CIHI and CCHSA
    regarding identification of nationwide indicators
    of patient safety
  • Explored electronic health record models in
    Capital Health and offerings by Eclipsys
    determined that CPSI will partner with CHI with
    CHI taking lead role

is 1.1.1 Continue to meet with key partners to
establish short, medium and long-term

and opportunities regarding development of a
national reporting system
2005/2006 Key Initiatives
Priority initiatives during 2005/2006 will
include 1.1.1(a) Publish a national report based
on stakeholder consultations outlining key
issues, initiatives and recommended priorities
for CPSI 1.1.2(a) Make significant progress
towards formal agreements with key national and
provincial stakeholders (e.g. CCHSA, CIHI,
CHI) Other Promote forums for discussion (e.g.
Halifax 5 and ISQua) and demonstrating progress
in major patient safety areas 1.2.1(a) Work with
legal/regulatory committee to establish and
implement a consultation strategy with
provincial, federal and regulatory stakeholders,

(2) an improved website, including a
comprehensive inventory of leading

system stakeholders to conduct root cause
analysis courses for health professionals /

the Root Cause Analysis framework (Manitoba
pilot) 2.2.2(a) Develop a communication and

of simulation-based training models, and to
promote these tools for interdisciplinary team
education, learning and certification programs

    the English version of the Canadian Patient
    Safety Dictionary
  • Provided support for the development of a French
    version of the Canadian Patient Safety Dictionary
  • Retained a legal consultant to complete overview
    of legal and regulatory issues in Canada
  • Initiated collection of existing disclosure and
    communication guidelines, and held discussions
    with provincial and national governments as well
    as health care organizations regarding the
    development of a national policy guideline
