• 152 FHTs funded in 3 waves various stages of
  implementation
• 50 more to be funded in next 2 years
• Usually 3 - 25 family physicians across practice
  sites
• 3 types of governance (provider, mixed,

  to serve orphan patients

• 36 Teams - 33 FHTs, 2 CHCs, 1 SCPI
• Second Collaborative begins November 3-4, 2008
• 37 Teams - 36 FHTs, 1 SCPI,
• Third Collaborative begins March 25, 26, 2009
