Collaborative Roll Out

  • Opportunity to send team offered to all FHTs
  • Up to 50 teams in each collaborative
  • First Collaborative began May 25-26, 2008
  • 36 Teams - 33 FHTs, 2 CHCs, 1 SCPI
  • Second Collaborative begins November 3-4, 2008
  • 37 Teams - 36 FHTs, 1 SCPI,
  • Third Collaborative begins March 25, 26, 2009
