What do we know?

• Health and Social care delivery will radically
  change in the next 10 years
• The PDSI Partnership has a part to play in the
  shaping of services for Disabled people living
  and working in Gateshead
• There is to be a rationalisation of all of the
  Partnerships in Gateshead
• There needs to be a clear reporting structure for
  the Partnerships to feed into
• Partnerships need to be accountable for their
  agreed priorities and outcomes

• Public Health to engage with a targeted
  preventative strategy, e.g. improved bowel,
  cervical and breast screening for women who are
  wheelchair users

  people in transition through the moving up
