The teams and the project work
  • Various regional teams some that have been
    established from the start and others that have
    recently come on board.
  • North Staffs - project work establishing links
    between older adult services and acute health
    services. The project resulted in a programme of
    education across the trusts, the development of a
    toolkit for use in a variety of clinical areas
    and, a publication is in the pipeline. A
    Chairmans award was received for the work which
    is ongoing. South Birmingham Mental Health Trust
    are looking at developing similar work and have
    just signed up!

The teams and the project work
  • Cannock Chase District assistive technology
    work supporting carers and people with Dementia
    to live independently for longer at home. Service
    called Just Checking uses movement sensors in
    the home.
  • Alzheimers café opening Cannock December 6th,
    2007 there are three open in the Staffordshire
    locality with another 3 to open.
  • Extra care housing potential for development in
    the locality.

The teams and the project work
  • South Staffs Council/Alzheimers Society/CASS
    Dementia Support Worker based in the Community is
    now having a real impact since its launch in late
    September 2007 and Age Concern Stafford and
    District are thinking about establishing a
    similar scheme so have signed up to the
    collaborative to take the work forwards.
  • Wolverhampton joint health and social care
    project around the patients journey, work is
    progressing towards a publication and action
    points have been agreed as a result of the
    questionnaires issued and issues raised from the
    patient journey work.
  • Stoke City Council will be signing up to take
    forward their Changing Lives work around
    service redesign with older people with mental
    health issues and their carers.

Where now?
  • Conference to showcase the work of the
    collaborative and celebrate what has been
    achieved by all in 2008.
  • Publications.
  • The WMOPMH Collaborative website and its renewed
    / sustainable development.
  • Continued development of In Our Shoes a module
    designed with service users and carers and taught
    by service users and carers running from January
    2008.
