2Background the need for an Outcomes Framework
- Spring 2006
- Consensus for some time that KPIs do not really
tell us enough about outcomes for service users - Need to move away from processes inputs to a
focus on outcomes - Concern that we need to be better able to
demonstrate what the public investment in SP
really delivers, both nationally and locally and
for individuals - Agreement on the need for a consistent approach
- National Outcomes Framework needs to measure SP
contributions to wider government objectives - Framework needs to evidence the wider overarching
outcome of supporting enabling independence
3Background the development of the Outcomes
Framework
- Summer Autumn 2006
- CLG Outcomes Working Group set up May/ June
2006 includes AA reps from - each region providers
- Outcomes framework begins to take shape
modelled on the 5 high level outcomes - from Dfes Every Child Matters Programme
- Detailed Indicators developed to underpin each
high level outcome - Requires a needs based support plan to be in
place for each user - Modelled on the Client Record Form
- to be completed for individual users at the point
of departure from the service for all short term
services - to be completed for individual users as part of
the annual review of the support plan for long
term services - Great debate on hard versus soft outcome
measures - Still need to build in flexibility locally
still to develop local basket of indicators

- Overarching SP Outcome is supporting enabling
independence - Five outcome domains
- Achieve economic well being
- Enjoy and achieve
- Be healthy
- Stay safe
- Make a positive contribution
5Background Comprehensive Spending Review/ PSAs
- Central Government currently agreeing new
National Set of PSAs Indicators - Huge reduction in terms of number of indicators
- Currently 1200 indicators collected across
government - Reducing to 200 by April 2008
- Crucial that SP is included as an indicator as
this is the mechanism to be - part of the new Local Government Performance
Framework - SP Outcome data to help demonstrate local
targets within Local Area - Agreements

6Overview the Outcomes Form for Long term services
- Short Walk through the Outcomes Form for Long
Term Services - Need to understand that this approach is based
on the expectation that individual - service users will have a clear, needs based
support plan in place which - identifies key areas where they need support
- The intention is to capture the actual outcomes
achieved for the user where they - have an identified support need
- There is also the ability, throughout the form,
for the provider to indicate why the - relevant actual outcome was not achieved
this could be for reasons relating to - the user, reasons relating to the service or a
range of external factors
7LongTerm Services Form - Provider, Service and
User details

- One outcomes form should be completed for each
user currently receiving long term support. - Captures data relating to the Provider, service
and characteristics of the service user - Provider IDs (5 digit National Client Record
Provider ID - 8 digit National Provider ID)
- Provider name
- Service name, service ID, service type, AA
- Client characteristics such as age, gender,
economic status, ethnicity, faith, disability,
applicable client groups - Then we move onto the 5 outcome domains..
8Standard format for each domain
- Questions under each domain follow a standard
format - This format is
- Did the person need support in relation to a
particular area/ activity? Y/N - Actual Outcome achieved? Y/N
- Is Ongoing Support required? Y/N
- If the Outcome was not achieved, why not? Various
reasons can be given here factors to do with
the client, the service or wider external
factors. More than one can be used.
9 Outcome domains 1 Economic Well Being its
indicators
- This outcome domain aims to capture the number of
service users who are being supported to achieve
measurable improved outcomes in relation to one
or more of the 3 support needs listed below - Support in relation to maximising income
- Support in relation to managing any debt, with
the actual outcome of having reduced their debt - Support in relation to paid work, with the
actual outcome of being in paid work
10Outcome domains 2 Enjoy and achieve its
indicators
- This outcome domain aims to capture the number of
service users who - have been supported to achieve measurable
improved outcomes in - relation to one or more of the 4 support needs
listed below - Support in relation to training and/or education,
with the actual outcome of - i) having participated in the desired
training/education and - ii) and if applicable, has the client
achieved the qualification - Support in relation to leisure/cultural/faith/info
rmal learning activities, with the actual outcome
of having participating in the chosen activities - Support in relation to work like activities e.g.
unpaid work or work experience with the actual
outcome of having participated in the chosen work
like activities - Support in relation to contact with external
services, groups, friends and family, with the
actual outcome of having made contact with the
desired people

- This outcome domain aims to capture the number of
service users who have been supported to achieve
measurable improved outcomes in relation to one
or more of the 4 support needs listed below - Support in relation to primary health care, with
the actual outcome that the person is better
managing their physical health - Support in relation to mental health services,
with the actual outcome that the person is better
managing their mental health - Support in relation to drug and / or alcohol
services with the actual outcome that the person
is better managing their drug/alcohol use - Support in relation to assistive technology /
aids and adaptations with the actual outcome that
the person is better managing independent living
12Outcome domains 4 Staying safe its indicators
- This outcome domain aims to capture the number of
service users who have been supported to achieve
measurable improved outcomes in relation to one
or more of the 5 support needs listed below - Support in relation to maintaining their
accommodation and/or avoiding eviction, with the
actual outcome of having maintained their
accommodation until the point of departure from
the service - Support in relation to improving their
compliance with statutory orders and related
processes, with the actual outcome of having
complied with statutory orders processes - Support in relation to self harm with the
outcome that the person is better managing self
harm - Support in relation to risk of harm to others
with the outcome that the person has avoided harm
to others - Support in relation to risk of harm from others
with the outcome that the person has minimised
the risk of harm from others -

its indicators
- This outcome domain aims to capture the number of
service users who have been supported to achieve
measurable improved outcomes in relation to the
support need listed below - Support in relation to having more involvement
and control, with the actual outcome of having
achieved more control and involvement, either at
service level or within the wider community -

15How to submit outcomes information1
- On line web entry system
- This method is accessible through our website
www.spclientrecord.org.uk under Outcomes. In
order to send us Outcome information, providers
need a log-in and password. To obtain these,
providers need to visit the website at
www.spclientrecord.org.uk CROSS section, and
sign up for web entry. Please fill in the
details and submit the client record office will
then send the provider a log-in and password via
email. Once providers have these, they can fill
in the Long Term Outcomes Forms on line, under
Enter Outcome Form by clicking submit. If
providers have previously had a log-in and
password for either of the Outcome pilots or
currently submit Client Record Forms by this
method, the same log-in and password should be
used.

- Â Â Â Electronic submission using SP Digital
- Â
- SP digital software is available to download from
our website www.spclientrecord.org.uk in the
section SP Digital 2008/9. This method allows
providers to complete the forms on screen.
Providers then save the file and email it to us
as an attachment to outcomedata_at_st-andrews.ac.uk.
Providers will receive an automatic reply to
confirm that we have received the email. If
providers require any help with this please

- Submission using our specification for
providers own systems - Â
- This specification will be available as a word
document from our website www.spclientrecord.org.u
k in the section SP Digital 2008/9. This
method allows providers to submit Outcome forms
to us using their own system. Data should be
emailed as an attachment to outcomedata_at_st-andrews
.ac.uk.
