- Grantees with cohorts 2, 3, or 3A funds must
complete the Grantee Profile, APR, PA Grantee
Report - Each cohort is reported separately
- For Grantee Profile APR, centers must be
reported separately - All reports are due October 31, 2008
- Reporting covers programming during summer 2007
and school year 2007-2008

student results are reported only for those
students attending 30 days (a.k.a. regular
attendees). These results must be further broken
down into attendance categories - 30-59 days

- Prior to completing templates, grantees should
- Review FERPA legislation
- Review the directions for each template
- Identify a contact person who can respond to
questions about data safeguarding - To complete the template, enter grantee-specific
information where the blue text appears - It is not enough for grantees to complete the
templates. Data safeguarding measures should be
adopted!

entire evaluation plan, including required
performance measure/indicators and additional
grantee interests - BEFORE Using the Evaluation Framework

identify the performance measures, performance

28Teacher Survey
- Grantees must survey a classroom teacher for each
student attending 30 days. A reading or math
teacher is recommended. - Grantees have 3 options for fulfilling the
Teacher Survey requirement of the APR - Paper-based survey distributed to and collected
from classroom teachers - Distribute a web link to classroom teachers so
they can complete an online version - Combination staff collect paper-based surveys
and enter them into the online system - For grantees using the web-based survey, AIU will
generate a report with a summary of the teacher
survey results and the raw data.

- Provides grantees with useful questions for
collecting feedback from stakeholder groups such
as parents, school administrators, students,
center staff, and partners - Includes Likert scale and open-ended questions
- Computes and graphs forced-choice survey results
